• Once scheduled, keep your appointment

When You Arrive:

• 1) Check in at the front desk with our staff
• 2) Have your insurance information with you
• 3) Be prepared to fill out health history, treatment consent and privacy forms for yourself and all family members who have an appointment

You have two options for completing these forms. You can either use our computerized Patient Check-in Stations, or we can print out paper forms for you to fill out by hand. Please let the front desk staff know which option you prefer.

Who is Eligable:

You must be actively enrolled in a Minnesota Health Care Program such as Medical Assistance or MNCare to receive services from Northern Dental Access Center.

* Please note that Northern Dental Access Center operates by appointment, with only limited availability and resources for emergency visits.
