Self-Pay Program - (Sliding Fee)
WHAT IS THE SELF-PAY PROGRAM?
It is an income-based sliding fee option for patients needing to pay out-of-pocket for dental care.
WHO IS ELIGIBLE?
People who have incomes that are considered low by federal and state guidelines are eligible for discounts on dental services. A simple verification process will make this determination based on household size and other factors. Uninsured people are also able to access self-pay options.
WHAT SERVICES ARE COVERED?
The Self-Pay Program applies to dental services available within the safety net scope of care currently available to all patients at Northern Dental Access Center.
HOW IT WORKS
Ask a Northern Dental Access Center employee for assistance. After a simple application process, you will be informed of the discounts available to you. Your proposed treatment plan will reflect your actual costs.
Dental services are provided regardless of race, color, national origin, disability, religion, sex, age and sexual orientation. Services are provided regardless of uninsurance or public insurance status.
WHAT IT COSTS
Discount levels are determined based on household size and reported monthly income. Verification of income levels may be required. In some cases, proof of current participation in other county services may suffice (i.e., housing subsidy, SNAP, fuel assistance, Head Start, etc.).
Payment for procedures is required in advance and is accepted in cash, credit or debit card.
Lack of payment can result in cancellation of your appointment.
Note: Any person falling within 250% of federal poverty guidelines is encouraged to enroll in a state health insurance program to assist with health care needs.

If you are unwilling or unable to verify exact monthly income - you will be charged the full cost of procedures.
