When You Arrive
- Have your insurance identification information ready before you call or ask about our cash-pay program if you do not have insurance (income eligibility guidelines apply)
- Call Bemidji or Halstad or stop either location during normal business hours
- Once scheduled, keep your appointment
- Ask about flexible appointments if your schedule is difficult to predict
When You Arrive
- Check in at the front desk with our staff
- Have your insurance information with you
- Be prepared to fill out Medical History, HIPAA and Treatment Consent forms for yourself and each family member who has an appointment
Cancellation Policy: A 24 hour notice is required if you cannot make a scheduled appointment. Because we have so many people waiting to be seen, if you miss three appointments without notice, your scheduling options may be limited.

- Be enrolled in a Minnesota Health Care Plan such as Medical Assistance or Minnesota Care (North Dakota Medicaid is accepted at our Halstad location). With your member number, you can contact us to make an appointment anytime.
- Meet income guidelines for access to a non-enrolled patient fee schedule. To find out if you're eligible, contact either location at the linked pages above.
